There are several useful resources for parents that include links, articles and forms.
Forms
These forms are important and we ask that each youth have them on file. Please click on the name of the form to download the form and return it to the church office.
Medical Release Form- Any youth partipcating in events needs to have this form on file.
Media Release Form- This form allows the church to use digital media to enhance the youth website or promote events. No names will be posted with pictures or videos.
